Health Checks Support Officer
The learning Disability Partnership (LDP) is offering an exciting opportunity to work within our integrated Health and Social Care teams to support us in the delivery of the Direct Enhanced Service (DES) to ensure that all people with a learning disability are offered an annual health check by their GP.
The post holder will work collaboratively with General Practices and professionals within the LDP to promote the uptake and delivery of the DES in GP practices, ensure registers are validated, learning disability awareness training is delivered and performance targets on completion and recording of health checks are met.
The role will involve arranging a programme of support for all Cambridgeshire General Practices in the LDP area this will include regular GP liaison and joint working with LDP professionals. The post holder will be responsible for the collation and reporting of activity data to a number of internal and external stakeholders both in written reports and attending meetings as required.
The successful person will be self-motivated and enthusiastic with excellent communication and IT skills and will have experience of developing effective working relationships with colleagues from different organisations and professional backgrounds.
