The integrated Health Visiting and Family Support teams deliver the Universal Healthy Child Programme and targeted interventions for children and their mums, dads and carers, from pregnancy to age five years. The emphasis is on prevention, early intervention and whole-family assessment, using the common assessment framework (CAF). Through universal screening, the Health Visiting teams identify children, mums, dads and carers with additional needs and provide support to improve outcomes and reduce inequality. The multi-agency teams consist of Health Visiting and Family Support as well as a range of multi-disciplinary expertise, including CAMHS, Adult Mental Health Services, Speech and Language Therapy, Welfare Rights Advice, Employment and Training support and Housing advice.

Our Vision: children reach a good level of development at age 2 and are ready for school by age 5.

Our Aims Are:

To ensure there are high levels of engagement in the service offer and sustained participation, with measurable impact. To deliver high quality services, ensuring families receive excellent customer service and consistently report positive experiences of using services. To ensure children are safe from harm, escalating to Children’s Services and Social Work (CSSW), when the level of risk and need meets thresholds for social work intervention.

Join our multi-agency Family Support team and provide family support to families with children from pregnancy to age 5 years.  We are seeking skilled and confident Family Workers, with an understanding of early child development, and who enjoy building positive relationships with parents, using a strength based approach. You will be able to work in partnership with families as well as other professionals within our multi-agency teams and the wider team around the family. You will carry out home visits and manage a varied caseload. You will act as Lead Professional, undertaking whole family assessments, creating a family action plan and coordinating Team Around the Family meetings.
