The Post

You will need to be passionate about serving our customers, have excellent communication skills and the creativity to find solutions that deliver excellent services our customers deserve. 

You will be responsible for assessing the needs of vulnerable adults and their carers. The job will require you to promote service user independence and produce robust outcome focused support plans; identify, manage and support positive risk taking and will require working in partnership with other statutory and non-statutory agencies to provide seamless service. 

The Person 

Will have the relevant professional qualification in Social Work. 

The person will need to have substantial experience of working with adults who have physical disabilities and older people with complex needs and understand the statutory framework which govern social care in England. 

The person will need to be able to work positively as a member of the multi-agency team on complex cases involving mental health, voluntary organisations and NHS/CCG partners. 

The person will need to be able to devise Care packages and Care Plans, Court and other reports, including assessing, monitoring and reviewing packages of care as required by the Care Act 2014. 

The person will need to be able to purchase Care Packages within Budgetary Framework.
