The Adult Social Care Integrated Triage Hub provides a multi-professional screening service for all referrals received by Bexley Adult Social Care using a ‘see and sort’ solution-focused model of intervention.    

The Hub is comprised of social work, occupational therapy, physiotherapy and nursing and is managed by a Service Manager, Senior Social Worker and a Senior Occupational Therapist. 

The Triage Hub provides a Duty response for the Complex Care Teams and CART.  Working closely with Health colleagues and the Rapid Response Team, the Triage Hub will deal with crises and emergencies as they arise.

We will look to you to provide a professional and personalised occupational therapy service to adults and elderly residents with physical disability and dual diagnosis of physical disability and mental health issues. The successful candidate will assess each individual case and recommend rehabilitative solutions that promote independence and improve the individuals’ quality of life, wherever possible.

At the London Borough of Bexley we are committed to embracing change and innovation you will be an integral part of shaping the occupational therapy service by continuously evaluating it on a professional and administrative level and providing recommendations for improvements on service delivery.

You will play a key role in the assessment and treatment of people, caseload management, supervision and training of junior staff as appropriate, complete Access / Home visits, support other Team members as appropriate and attend family and MDT meetings. 

Due to the fact that you’ll be carrying out visits in patients’ homes and other settings within the community, it is essential that you are able to travel around the Borough.
