The Post 

We have recently completed our restructuring process and now looking to fill the posts that have become available across Adult Social Care teams that include:

• Hospital Discharge 
• Integrated Reablement and Rehabilitation 
• Mental Health 
• Safeguarding 
• Community Review 
• Commissioning, Contracting and Market Management 

Successful candidates will be offered a choice among these teams, to support our vision of improving health and well-being outcomes for our residents, working in partnership with the local health service and the independent and voluntary sector to provide personal and practical support to help people lead full and active lives, maintain their independence and dignity with choice and control.  

The Person 

We are looking for Qualified Social Workers (who are registered with HCPC) with excellent communication skills, experience in carrying out assessments and support planning, especially within a fast paced team. 
This role will offer you the opportunity to develop your multi disciplinary working skills as well as enhancing your negotiation and organizational abilities. 

You will have supportive managers and you will work within a dedicated team of colleagues with good knowledge of the discharge process.
