As an experienced, qualified professional, you will act as a role model, playing a key part in promoting personalisation, independence, wellbeing and choice by providing a social care service to people which will include carrying out the most complex asset-based assessments / reviews, short term enablement plans (STEPs), care and support plans and safeguarding plans.

You will offer ongoing advice in line with the principles of SLCC / Signs of Safety and Wellbeing, promoting self-support, reablement / progression and personalisation; where necessary, provide daily living equipment, adaptations, assistive technology, community and other resources to promote independence and wellbeing.

This is an exciting time for Adult and Community Services, having gone through a re-design of our social care service, putting in place locality and partnership working and enhancing our integration with health.

A relevant social work qualification and HCPC registration is essential, along with continued professional development. You will be required to travel frequently in rural areas, therefore you must hold a full driving licence and have access to personal transport.

* The salary range is in line with our ACS Social Work Services Progression Scheme.
