The Role

This is an exciting time for our Learning Disabilities and Autism teams within Adult and Community Services as we re-vamp the service and improve outcomes for people.

We are currently recruiting following a redesign of Social Care Services, putting in place locality working and pushing forward with integration. As part of this redesign, we have invested additional capacity into our Learning Disabilities and Autism teams and introduced more qualified social worker roles and new Occupational Therapists' opportunities.

We have brought operational delivery and service development together in order to push ahead with our transformation. We would like to invite you to join our strong team and play a part in creating best practice for Suffolk, developing new pathways and ways of working.

You will enjoy the opportunities that our new service can offer, working closely with teams across Health and Social Care including NSFT (Norfolk and Suffolk Foundation Trust) and CYP (Children and Young People's Services).

Co-production is at the heart of what we do, and there is a strong core of Professional Advice / Practice Development.

You will work to our SLCC strategy and Signs of Safety and Wellbeing practice model, promoting independence and self-support, short term enablement, personalisation and digital care.

A relevant social work qualification and HCPC registration is essential, along with evidence of continued professional development.

You will be required to travel frequently in rural areas, therefore you must hold a full driving licence and have access to personal transport.

Salary ranges are in line with our ACS (Adult Community Services) Social Work Services Progression Scheme.

For an informal discussion only, please contact Julia Wunderlich on 07795 952745.

Please read the Job and Person Profile before applying.

Please indicate in your application which Grade you are applying for.
