An exciting opportunity has arisen in the Hospital Discharge Team (HDT) for an experienced Social Worker who is registered with the HCPC.   
 

Brent Hospital Discharge team is responsible for facilitating safe and timely discharge of Brent residents from all acute hospitals and works closely with other members of the multi-disciplinary teams from major hospitals for example Northwick Park, Central Middlesex, Royal Free and the Imperial hospitals.

The Post

The Deputy Team Manager will work alongside and support the Team Manager in providing specialist advice, guidance, support and supervision to Qualified and non Qualified Social Work staff within a fast past paced and multi-disciplinary hospital discharge service. The Deputy Team Manager will be dealing with day to day quality assurance, allocation, customer care and performance issues to manage the delayed transfers of care and reporting to the Team Manager as and when required. 

This post is with the Hospital Discharge Team (HDT), which is responsible for facilitating safe and timely discharge for residents of Brent and working closely with other members of the multi-disciplinary teams from the local acute and non-acute trusts that include Northwick Park, Central Middlesex, Royal Free and the Imperial and Willesden Centre for Health and Care. 

The Person

You will have substantial experience of managing and supervising qualified and unqualified Social Work staff. 

You will have experience of working within a partnership and multi-disciplinary setting would be advantageous. 
You have the ability to risk assess referrals into the team, planning a response and deciding on priority.

Skills in multitasking, prioritisation, workload management and time management are all essential.
