The Role

In partnership with other NHS, Local Authority and Voluntary Sector providers, Camden and Islington NHS Foundation Trust will be introducing a Primary Care Mental Health service across GP surgeries within the London Borough of Camden. This is an exciting opportunity to join the new service as it begins to develop in to the new primary care mental health team currently based in the St. Pancras hospital site.

The local authority have secured some extra ‘winter’ funding which means we are looking to recruit 2 fixed term Social workers in an exciting and new primary care mental health service across GP surgeries within the London Borough of Camden. We are looking for 2 experienced social workers who are keen, enthusiastic and passionate about social work and primary care working to join us on a two year fixed term contract.

This role will involve providing mental health and care coordination to service users within the 24hr hostel pathway in Camden. The team are a multidisciplinary service, offering care- coordination, crisis interventions and move through to this cohort of community patients, who will have a diagnosis of psychosis and require intensive support to reach the optimum level of wellbeing.

The team is a two year funded project and continued funding for the team will be dependent on the successful implementation of the service

The successful candidate will need to be a qualified Social Worker, HCPC registered with experience of working in the community supporting adults with mental health problems following the Care Programme Approach (CPA). The successful candidates will be enthusiastic, passionate and committed to work within the recovery model and dedicated to improve the well-being of service users and their carers.  It is essential that you demonstrate the ability to take responsibility for managing your own workload at a high standard and delivering to set deadlines, despite tight timescales and conflicting priorities. You will demonstrate a sound understanding of the role as a Care Coordinator and join our friendly multidisciplinary team of Social Workers, Occupational Therapist and Mental Health Nurses who are experienced community practitioners. The team also comprises of Assistant Practitioner, Clinical Support Workers, Psychologists, Psychiatrists and Therapists, supported by our admin staff. 

You will have experience of building and maintaining effective working relationships at all levels internally and externally to high service standards. You will have strong awareness and understanding of the complexities of operating in a multi-disciplinary setting.  You will also be proficient in using Microsoft Office Packages i.e. PowerPoint and Word, Intermediate Excel. Have excellent communication skills both written and verbal.

This post is exempt from the Rehabilitation of Offenders Act and will require an Enhanced DBS check.
