An exciting opportunity has arisen for an experienced Social Worker to join our fast paced, dynamic and evolving multidisciplined Enablement team based in Wandsworth.

The successful candidate will work closely with the multidisciplinary team to ensure that the service promotes independence in line with the Richmond and Wandsworth's values and behaviours. The role will require that the candidate has a relevant qualification in Social care as well as being HCPC registered.

The successful candidate will:

  • Provide Case/Care management.
  • Undertake effective functional assessments of the individual/carers needs and risks to encourage independence, provide support and prevent harm.
  • Make Recommendations and set goals to enable function.
  • Address identified needs to promote well-being in accordance with the Care Act 2014.
  • Work as a trusted assessor in relation to equipment and minor adaptation provision.
  • Provide advice and support to multidisciplinary colleagues.
  • Have at least 2 years post qualification experience.

This is a busy team with a fast throughput of work, therefore excellent time management skills and the ability to manage your own caseload is essential.

The selection process will include an interview and a written test.
