The Hospital Integrated Discharge Team is a multidisciplinary team which works to ensure the safe discharge from hospital of Bexley residents. The team have staff based in Queen Elizabeth Hospital Woolwich, Darent Valley Hospital Dartford, Princess Royal University Hospital Farnborough and Lewisham Hospital. Whilst mainly based in Queen Elizabeth Hospital you will provide outreach to the other hospitals in the group.

We are looking for Social Workers who are able to work in a multi disciplinary team alongside  nursing, therapy and social care colleagues. You will  be assisting the Team Manager in providing a comprehensive integrated service, safeguarding vulnerable adults and promoting safe discharges from hospital for Bexley residents including assessing them in hospital, in placement or in  their own homes following discharge from hospital.

Appointment to this post will be subject to the outcome of an Enhanced Disclosure obtained through the Disclosure and Barring Service.
