You will be part of a busy team which works closely with the multi disciplinary team to facilitate timely and safe discharges from hospital.

The work is varied and challenging but the role is key to ensure people achieve their outcomes.

This is a great opportunity to gain experience of Hospital Social Work and enhance your professional development.

You will be working with adults and carers to deliver Self-Directed support and Self-Assessment as well as safeguarding, adopting a positive approach to risk.

You will join a busy but supportive team.  We pride ourselves on supporting staff through their career including ASYE and level 3 social worker progression. It is an exciting time in the team as we continue to develop and adapt to progressive ways of working. Whilst we need to be visible within the hospital we do offer mobile and agile working meaning there are opportunities to work from home.

There will be a strong focus upon your own personal professional development to enhance your skills and practice.  Our aim is to develop confident and competent practitioners, thereby supporting better outcomes for people we work with and equipping you with real and transferable skills for the future.

You must hold a HCPC recognised Social Work Qualification and be registered with the HCPC.

Travel is an essential part of this job and therefore it would be extremely difficult for you to do your job effectively if you did not use a car for all or most of the time.  It is essential that you hold a full and valid driving licence and it is expected that you will either use a council vehicle or your own car.

An enhanced Disclosure & Barring Service Check (DBS) will be required for this job. A conviction may not exclude candidates from appointment but will be considered as part of the recruitment process.
