Are you looking to be part of an innovative, award-winning Independence team? We have an opportunity for a Part Time Social Care Assessor within the Independence and Reablement Team in the Ripon Boroughbridge and Knaresborough area. We are looking for a Social Care Assessor who is motivated and dynamic to join our team of experienced staff to deliver an effective holistic service.
This is an excellent opportunity to join our Independence and Reablement Service within Health and Adult Services and our inclusive way of working enhancing person centred care. Health and Adult Services is an exciting and friendly place of work, committed to delivering services to its local community and providing opportunities for our local resident’s. You will support the needs of the client establishing person centred goals, assess their short term needs, conduct timely reviews and progress clients within the service where required.
You will work to increase the client’s independence and quality of life, maximising function to empower and enable them to remain in the community for as long as possible. You will work closely in partnership with a variety of teams and agencies, including health colleagues, stronger communities and prevention teams, voluntary and independent sector, Mental Health and public health colleagues. Our ethos is to promote health and wellbeing, preventing, reducing and delaying need, in line with the Care Act.
As a Social Care Assessor, you will undertake a range of strength based assessments and interventions to ensure that adults with care and support needs and their carers achieve their desired outcomes and wellbeing to live independent lives. You will determine eligibility for adults with care and support needs against the national standards and ensure that they are safe from harm but where harm or abuse is identified ensure the safety of the person and undertake the relevant safeguarding enquiries.
To be eligible for this post you will have a professional Degree, and have current registration with the Health and Care Professions Council.
