The Role:

We have an exciting opportunity available for an Occupational Therapist to join our Community Independence Service.

The Community Independence Service is an integrated health and social care service designed to provide support to adults with complex health and social care needs to keep them safer at home for as long as possible.

You will be joining a multidisciplinary team who are enthusiastic about promoting independence and setting goals with our residents to enable them to regain lost skills, acquire new ones and restore confidence following a crisis.

About You:

As a qualified Occupational Therapist you will be responsible for carrying out holistic assessments of our residents needs and setting Reablement goals with them.

You will show initiative, drive and determination to get the job done and are self-motivated to improve what you do.  You put our residents’ needs at the heart of the work that you do, listening to their needs and advocating for them when needed.

Role Requirements:

  • Complete assessments, support plans and reviews in line with the Care Act 2014 and ensure that residents are supported to make choices to meet their needs. Investigate opportunities for residents and carers that maximise their independence and seek out alternatives to council commissioned services.
  • Carry out strength based assessments that are proportionate and in line with the principles of well-being. Promote self-assessment tools to optimise residents involvement.
  • Apply a personalised approach, identifying those people suitable for rehabilitation, or reablement. Support with goal setting and utilise prevention services to facilitate independence and wellbeing.
  • Complete manual handling risk assessments, interventions and equipment provision; adhering to relevant manual handling legislation, local policies and procedures.
  • Work autonomously to manage your own clinical caseload. Provide clinical advice to cases managed by Social Workers and Independent Living Assessors.
  • Manage cases effectively and respond to residents’ needs in a timely manner within a fast moving and highly pressured environment.
  • Build and maintain strong multi-agency relationships. Working effectively and in partnership with other professionals from statutory and voluntary organisations to meet residents’ needs in a collaborative manner.

We work in partnership with Hampshire County Council (HCC) who deliver many transactional services on our behalf. Although most communications will be from HCC, all selection and appointment decisions will be made by RBKC. Certain services are shared across more than one Council area to nearby neighbouring Authorities. This enhances delivery and reduces costs. Your recruitment information will advise if your remit extends beyond RBKC.
