The Post

An exciting opportunity has arisen in the Hospital Discharge Team (HDT) for an experienced Social Worker who is registered with the HCPC.
 
Brent Hospital Discharge team is responsible for facilitating safe and timely discharge of Brent residents from all acute hospitals and works closely with other members of the multi-disciplinary teams from major hospitals for example Northwick Park, Central Middlesex, Royal Free and the Imperial hospitals.

The Person

We are looking for an experienced Social Worker with excellent communication skills, experience in carrying out Care Act assessments and creative support planning within the context of facilitating hospital discharges. The successful candidate will be able to manage complex cases in a timely manner to facilitate safe discharges for vulnerable adults.
 
This role will offer you the opportunity to develop working within a multi-disciplinary setting. You will have supportive managers and you will work within a dedicated team of colleagues with good knowledge of the hospital discharge process.

 
To be successful in this role you will have:
- Substantial experience of working in partnership and within a multi-disciplinary setting, within Adults Social Care and or Hospital Discharge.
- Managing well under pressure with ability to respond to urgent referrals.
- Skills in multitasking, prioritisation, workload management and time management.
- Will have experience with working with social care legislation and undertaking relevant assessments.
- Be passionate about working with vulnerable adults and providing an excellent service
