About the RoleAs Integration and Partnership Locality Lead, you will work with our operational teams and NHS and other partners to design and deliver the integration of health and social care. The role will support our four quadrant areas (each the size of a large London borough) in developing relationships with key partners, to identify best practice.A proven track record of influencing and the ability to build strong collaborative working relationships across the service to support operational delivery are essential in this role. Your responsibilities will also include:- Work on a locality basis, supporting the Director for Integration and Partnerships, Head of Integration and Partnerships and Directors for Local Delivery, to design, develop and deliver new models for integrated and collaborative working with NHS and other partners Lead on project delivery for locality integration projects, working with local partnersHorizon-scan of models of partnership working nationally and internationally to continually improve the service offer within Essex.Advise and support the quadrants in leading and developing local partnerships in Essex with key partners including, Clinical Commissioning Groups (CCGs), District and Borough Councils, NHS Community and Acute Trusts, voluntary and independent sector organisations, developing integrated delivery models to support the delivery of commissioned outcomes.Encourage innovation in service delivery, promoting community resilience and direct involvement, where relevant, in delivering effective targeted intervention services; horizon scan of models of partnership working nationally and internationally to continually improve the service offer within Essex.Ensure consistency across the County, sharing best practice on partnership work and integration models.
