About the RoleThis is a chance for an experienced Occupational Therapist to take a step forward in their career or to expand their knowledge and experience in working within a Discharge To Assess Model.This model of practice is ever evolving and developing and as a member within the team you will be an influencer of change and progression.You will be supporting adults who have recently been discharged from hospital or supporting them to prevent an admission into hospital. The ethos being to support Adults to leave hospital, when safe and appropriate to do so, and continuing their care and assessment out of hospital.Wherever possible Adults will be supported to return to their home for assessment. Your support will be focussed on enabling adults to remain as independent and self-caring as possible in their own homes. However, some Adults may have been discharged to a temporary care home environment to aid their recovery and for a period of assessment to be able to identify their short or long term care and support needs.You will be working in collaboration with allied health care professionals and care providers using a multi - disciplinary approach to ensure the adults receive the right support at the right time.As an Occupational Therapist, you will be supporting the development of practice, skills and knowledge with the Social Workers and Community Support Workers within the teamYou will primarily be based in Mid Essex however; you may be asked to travel outside the county in line with operational demand.A key part of the role is the ability to work alongside families, ECC category and contracts colleagues, ELS legal services, our health partners and providers to promote opportunities for our adults to maximise their progression and independence with a person-centred approach About YouTo apply, you'll need a degree or diploma in Occupational Therapy, CQSW, CSS or equivalent qualification, with current Health and Care Professions Council (HCPC) registration. With experience of working in a Social Care or Health setting ideally working to support vulnerable adults. This should include the ability to work with a complex caseload underpinned by evidence-based practice.This role involves travelling around Essex. A driving licence and car - or the ability to meet the mobility requirements of the role through other means - is essential.All candidates applying to Adult Social Care will be expected to join on a 12-week rota on a 6-day shift pattern and this will include some weekend work. This post attracts a 6% pay allowance for working Sundays and Bank Holidays on a rota pattern over and beyond the 12 week rota.
