We are looking for a social worker or senior social worker to join our 0-25 Disability Service and play a central role in delivering high-quality and responsive social work across children’s and adults’ services (up to 25).

It’s a role to assess, plan and review disabled young people for short breaks, wellbeing and safeguarding purposes under the Care Act. The role will see you work collaboratively with allied health and social care practitioners, bringing together and coordinating the team around the young person to ensure a multi-disciplinary approach to safeguarding, as well as Assessment, Planning, Implementation and Review (APIR).  

You’ll act as the lead professional in statutory cases - including Transition (Care Act 2014) assessments and support planning, mental capacity work and safeguarding adults - while undertaking the full range of social work tasks with disabled young people and their families. 

Ideally the successful candidate will have knowledge and experience of working in both adults and children’s legislation. However, this is not essential and full training will be provided on areas that you’re not a specialist in.

  • A relevant social work qualification and Social Work England membership
  • Passed your ASYE year
  • Creative planning & Assessment skills
  • Experience of working across voluntary and statutory agencies
  • A excellent communicator
  • Understand the needs of children and young people
  • Applied knowledge of relevant legislation and guidance
