The SMART Service (Supportive Multi-Agency Response Team) is looking to recruit an experienced leader for a Team Manager post with a background in Occupational Therapy or Social Work.

The Post

This is a new and unique service in the London Borough of Brent and was launched in 2020 as a pilot project to provide targeted support for individuals with intersecting complex needs, particularly those with vulnerability and experience of exclusion that impacts on their mental health and wellbeing. These individuals would not normally be eligible for support from adult social care, as they do not meet the criteria set out in the Care Act. They may be under primary care for mental health, and are less likely to be managed long term under secondary mental health, potentially due to their reduced ability to engage.

The SMART Service is a creative response to provide a permanent multi-disciplinary service for people within the Borough who often have a cycle of homelessness, mental health admissions, drug and alcohol dependencies and contact with Criminal justice and probation services. Their multiple complex needs requires a more holistic response in order for them to be able to move forward and until now there has not been a targeted provision for this group of people.

The Person

We are looking for an experienced leader who has demonstrated a high level of expertise in working with people with mental health problems and complex social problems in the community. You will be able to maintain and further develop the quality practice and development of the trauma informed approach and strengths based practice of the team.

You will have demonstrable skills in developing service provision, to include making additional bids for funding, and evidencing and reporting the outcomes and impact of the service provision on the community.

The Team manager will provide clinical and management supervision to the Advanced Practitioner and up to 6 members of the MDT including Social Workers and/or Occupational Therapists, Housing Officer and Substance Misuse practitioners. You will attend and contribute to multi-agency forums and develop and nurture relationships within the Local Authority and across partnership agencies.

You will have a commitment to staff wellbeing and reflective practice.
