The Greenwich Older Adults Community Mental Health team is an established multidisciplinary integrated team, working to meet the needs of older people in the Royal Borough of Greenwich who experience mental health issues. We are keen to appoint people who share our vision of being truly person centred in our approach to care delivery.
You will be a social worker who has had experience or enthusiasm to work with older people with mental health needs.
You will be responsible for the assessment and care management of older people with mental health needs in the Greenwich area as part of a multidisciplinary approach to care.
You will act as a Care Co-ordinator, holding a case load of people for whom you will provide health and social care.
You will have an understanding of investigating safeguarding concerns, preparing cases for the placement panel, undertaking annual placement reviews, and be familiar with the use of electronic records.
Ideally you will be qualified and practising as an Approved Mental Health Practitioner or be willing to undertake this training.
