The Connect Programme is a large transformation project, delivered in partnership between Essex County Council and Mid and South Essex Partnership, and many stakeholders across the Essex footprint. There are 5 interconnected workstream, and the Programme will transform services for older people through better connections to the right support in hospital and at home.

Connect is now in an exciting design phase which will define what needs to change and how we can achieve the best outcomes for older people, through five inter-related projects. Over 60 frontline colleagues and managers are already involved in co-designing new ways of working.

Later this year, teams across the county will take what pilot teams have learnt, and the post holder will focus on supporting the three Essex County Council workstream leads (Supporting Independence, Discharge Outcomes and Reablement) and adapt them to suit their local area, ahead of county-wide adoption. The remaining two projects are being delivered across Mid and South Essex, in conjunction with our health partners: Admission Avoidance and Community Hospital Flow.
