In Kensington and Chelsea, we support people’s independence and wellbeing, and work hard to enable them to live in the community with dignity, choice and control. Our Community Independence Service (CIS) is a multidisciplinary team including community nurses, social workers, occupational therapists and reablement officers who work together to support adults with complex health and social care needs. We provide short-term support for people discharged from hospital, or recovering from illness or injury, to help them develop the confidence and skills required to remain independent at home.
At present, we are handling a high number of hospital discharges, but our team has the capability to flex according to community need. As a key member of our close-knit team, you’ll take on specific responsibility for strengths-based assessments: looking at each individual’s circumstances and helping them to plan support that is specific to their needs and will help return them to independence. As you focus on identifying risks and ensuring that there is appropriate protection in place, you will sometimes find yourself negotiating complex family dynamics, as well as initiating intricate networks of care.

Flexible, resourceful and collaborative in your approach, you’ll be ready to contribute to a hardworking and supportive team. A qualified and registered Social Worker, you’ll have a good understanding of social care legislation and safeguarding. In particular, you’ll be practised at doing care assessments, including mental capacity assessments. As you’ll be dealing with people who have new diagnoses or injuries, you’ll need to be sensitive and empathic. The ability to communicate effectively with a wide range of people, from fellow professionals to family members, is essential. You’ll also need to be comfortable working with electronic case management systems.
