About Us:

Offering support to people in their own homes

In Kensington and Chelsea, we support people’s independence and wellbeing, and work hard to enable them to live in the community with dignity, choice and control. Our Community Independence Service (CIS) is a multidisciplinary team including social workers, occupational therapists and reablement officers who work together to support to adults with complex health and social care needs. We provide short-term support for people discharged from hospital, or recovering from illness or injury, to help them develop the confidence and skills required to remain independent at home.

The Role:

Working closely with our Occupational Therapists and Social Workers, you’ll carry out basic assessments of people’s capabilities and help them find the motivation and support to progress. By getting to know people as individuals and understanding their specific circumstances, you’ll deliver person-centred care. Expect to manage your own caseload of service users, for whom you’ll recommend equipment, and set and monitor goals and complete care act 2014 assessments. You’ll also be alert to risks and to the need to involve other professionals. Although we’ll offer you the chance to work independently, you’ll have the full support of our close-knit community team.

About You:

This is an ideal development role for someone who is interested in a career in social care or occupational therapy, as it has the potential to lead to an apprenticeship and/or professional qualifications. You’re likely to have some relevant experience that’s given you an insight into issues around ageing and disability: you might have worked with a relevant charity, or perhaps as an OT Assistant or housing support worker. Well-organised and responsible, you’ll need strong communication skills and a willingness to learn and work as part of a team. Although we’ll provide full training in systems and processes, good Maths and English and some IT skills will be an excellent foundation.
