And we’re making radical social change a reality, so that nobody gets left behind. Here’s where you can help decide a better future for us all.

The role

The Children and Young People Disability Service 0-25 service is part of MOSAIC, the integrated service for disabled children who have complex developmental needs. Within this role you will focus on assessment and care planning of children and young people with severe to profound disabilities and complex health needs from birth to 25. You will supervise Social Workers and Social Care assessors within the service, you will be involved with managing Duty and supporting newly qualified social workers in their first year of employment.

We are looking for a talented, dedicated, highly motivated Senior Practitioner who is keen to progress and share our values of working as one team and finding better and more innovative ways of doing things. You will need to be a pro-active analytical thinker, with experience of working in an integrated setting with disabled children and young people, experience of preparing for adulthood work and experience of safeguarding and an understanding of the Children and Families Act 2014, Care Act 2014 and Preparing for Adulthood agenda.
