The post holder is designated responsible for supporting the Community Led Support Co–ordinator with the development, and co-ordination of Community Led Support working across  Adult Social Care

This role is key to supporting the delivery of a service providing strengths based interventions ensuring the individual and/or family receive holistic, individualised support focused upon promoting independence, and maximising use of community resources.

Supporting Community Led Support Co-ordinator to develop early intervention services within neighbourhoods.

The post holder will have knowledge and experience which will support delivery of services to individuals with wide ranging levels of need.

Have good knowledge of strengths based approaches, and promote an individualised model including Assessment, Support Planning, Personal Budgets, Direct Payments, assistive Technology etc.

The post holder will be confident in using Microsoft applications including Teams, Forms, Excel and Powerpoint. Managing the booking system for clients and rooms.

Ensuring that all Adult and Children’s legislation is adhered to including The Care Act, Safeguarding (both vulnerable adults and children), Mental Capacity Act and Deprivation of Liberty Safeguards.
