Job Details
The post holder will be responsible for a caseload of children and young people within the area of Preparing for Adulthood (PfA) with Special Educational Needs and/or Disabilities (SEND) who either have or are applying for an Education, Health and Care Plan. This includes responsibility for placement of young people, co-ordination of support services to deliver provision specified on an EHC plan and effective transition between phases of education and into adult social care.
