Haringey Adult Social Services is committed to achieving the highest standards in care and safety so that residents can look after themselves and live more independent and fulfilling lives.

We are now looking to a highly motivated Senior Practitioner to work in the Transition Team with specialist skills and knowledge in supporting adults with learning disabilities and autism. You will work within a multi-agency community team to ensure we deliver the very highest of quality integrated health and social care support to our residents and have a passion for supervising other social workers to help them develop their professional practice and their career development.

About the Role

As Senior Practitioner, you will be a senior member of the Team, providing expert advice and guidance, and ‘hands on’ practice to work through complex health, social care and legal processes and ensure people are supported safely in the community and signposted or connected with community resources who will continue to support them.

Your responsibilities will include complex care co-ordination which will involve interacting with professionals across health and social care teams in order to ensure that all residents have a high quality of care and support at all times. This will involve liaising with Haringey learning Disability Partnership and wider Council services, for example, Reablement, Housing, Community Safety, Brokerage, or Legal services. 

You will be expert in giving others advice and leading by example ensuring that Care Act assessments and Care Programme Approach reports reflect integrated health and social care, are well written in plain English, and embrace a recovery and strengths-based approach.

As a senior practitioner you will also assist the Head of Service and team managers to ensure that social workers are supported in their practice and offer a reflective space and professional supervision.

You will also be a lead and confident accessing and using Council resources and systems including MOSAIC, Hari-NET, Community Connectors, and systems that support social workers learning and development in Haringey as well as other national professional development networks.

Working in one of London’s liveliest and most culturally diverse boroughs, you will seek to maintain a strong partnership between London Borough of Haringey and Barnet Enfield and Haringey Mental Health Trust.

About You

You will be a qualified Social Worker with a social work degree and previous experience of a leadership role within a multi-disciplinary team and have a sound knowledge of legislation that underpins health and social care for service-users learning disability.  You will have a confident approach to a range of health and social care records, processes and systems.
