We are now looking to appoint 3 highly motivated Social Care Workers with knowledge in supporting adults with learning disabilities, and who enjoy working within multi-agency community teams to deliver the very highest of quality integrated health and social care support to help our residents.
About the Role
As a social worker you will be a senior member of the team, providing expert advice and guidance, and ‘hands on’ practice to work through complex health, social care and legal processes and ensure people are supported safely in the community and signposted or connected with community resources who will continue to support them.
We are recruiting 3 enthusiastic social workers to support very vulnerable clients with LD diagnosis to review clients’ care, review support plans within the strength based model of social care.
About You
You will be a qualified Social Worker with previous experience within a multi-disciplinary team and have a sound knowledge of legislation that underpins health and social care for service-users with autism, learning disability and mental health needs. You will have a confident approach to a range of health and social care records, processes and systems.

Working in one of London’s liveliest and most culturally diverse boroughs, you will seek to maintain a strong partnership between London Borough of Haringey and Barnet Enfield and Haringey Mental Health Trust.
