Do you feel that you can work to deliver the outcomes of personalisation?

The Slaithwaite Community Wellbeing Locality Hub enables care and support to be provided for people aged 18 and over with complex health and social care needs. They work to manage the changing care and support needs of Adults and Carers through the completion of Person Led Assessments and planned and unplanned reassessments, promoting a strength based and person led approach to practice. Your work will also include Safeguarding, self-neglect and Crisis Intervention, adopting an approach of positive risk taking. You will be expected to support as a Duty Worker on a rota basis.

You will be working with Adults and Carers to encourage Self-Directed support and Self-Assessment.

There will be a strong focus upon your own personal professional development to enhance your skills and practice. Our aim is to develop confident and competent practitioners, thereby supporting better outcomes for people we work with and equipping you with real and transferable skills for the future.

You may be required to work across different teams to meet the needs of the service.

Our offer to you:
