We are looking for Social Workers who share our values and vision for integrating health and social care and discharge to assess for the each of the following teams:

Richmond Response & Rehabilitation Team (RRRT social work) Wandsworth Hospital Teams (St George’s Hospital Team and Out of Borough Team).

Hospital social work often requires a response to crises, so the ability to think quickly, act calmly and work creatively is needed in this area of social work, to facilitate safe and timely discharges. We are committed to positive outcomes, ensuring that our residents and carers receive the right care, in the right place, at the right time.

You will be required to undertake Care Act assessments, reviews and support plans. Candidates must have post qualification experience along with a strong knowledge of relevant legislation including Mental Capacity Act 2005, Safeguarding Adults, Deprivation of Liberty Safeguards and how to apply this legislation in relation to hospital discharges. You must have good, clear communication skills, sharp decision-making and time management skills.
