Social Prescribing works with people in a holistic way to help empower them to take better control of their health and well-being. It provides people with the time to talk about ‘what matters to them’, connecting them to community-based activity and statutory services that provide practical and emotional support.

You will work with a wide range of people, their families and carers to help them to take control of their wellbeing, live independently and increase involvement with local communities. You will be required to build trusting relationships, using an asset and strength-based approach and provide personalised support to help people and their families to live well.

You will work closely with the voluntary and community sector to ensure people are able to access provision that meets their needs and help to increase the strengths and capacities of local communities.

You will primarily be working within the Viaduct Primary Care Network which is made up of eight GP practices in Huddersfield.

This opportunity can be done as a secondment.
