The Role
A Community Assessment Officer (CAO) opportunity has arisen to join North District Adult Social Care Team working with adults and older adults. We work with adults from 18+ till end of life. Due to this the work is varied and multi-faceted.
As a CAO some of the tasks you will be completing are:
Detailed assessment of need (Care Act) and effective asset-based support planning, taking into account available resources, universal and voluntary services
Identifying and managing risks
Completing Carers assessments
Mental capacity and Best Interest work
Preparing for and participating in decision-making forums and multi-disciplinary meetings.
These tasks should be completed in accordance with legislation, policies, procedures and best practice.
The role also includes covering a busy duty service, taking referrals and providing advice and guidance to members of the public and other professionals.
About You
We are looking to recruit an enthusiastic and motivated individual who has a passion for serving Adults in Bolton specifically in North District. We are looking for an individual who will demonstrate understanding of their needs, aspirations and supporting them to enhance their quality of life.
You will have the ability to develop and maintain relationships with team members, external organisations, service users and carers. You will have the skills to be actively involved in multidisciplinary team working and decision making.
You will be able to remain focused and motivated during current remote working arrangements that are in place.
These posts are subject to enhanced clearance from the Disclosure and Barring Service.
