An exciting opportunity has arisen to work to assist in the coordination to support the management of a comprehensive countywide Team Support Service across A&C localities whilst maintaining a high-quality responsive customer service. You will be working across A&C Locality Teams and will be supporting Working Age and Older Adult Social Care Teams to deliver the Target Operating Model (TOM).

The successful applicants will be responsible for supporting the TOM and establishing positive relationships, to include ASC Service Managers and Care Pathway Teams, ASC Finance, Business Support Managers, Admin Team Leaders, ICT, Health & Safety teams.

You will be motivated to take forward service improvements, deliver high quality performance, and service delivery within the responsibilities of the role.

The role will involve taking inbound and making outbound calls, arranging appointments for our Support Workers to visit the homes of service users. You will also liaise internally with other departments and update information on our internal database.
