Job Details

City of York Council Adult Social Care (CYC ASC) are excited to launch our new Specialist Designated Practitioner (SDP) roles, fixed term for 2 years. The SDP will provide targeted support to ensure those on the Dynamic Support Register (DSR) have safe and timely discharge plans in place, while those at risk of inpatient admission receive early intervention and preventative support to avoid admission. The SDP will support the integration and co-ordination of interventions within the Transforming Care Programme (TCP).

With a minimum of two years post qualifying experience, this is an opportunity for the successful candidate to further skills and expertise in a specialist area of Social Care within a committed and multi-disciplinary, multi-organisational team. We’re keen to hear from professionals registered with Social Work England or equivalent who want to provide hands on care management support and co-ordination to people whose care and support needs have a high degree of complexity, while providing leadership and support to colleagues across Adult Social Care working with the same cohort.

Your work will be underpinned by a comprehensive understanding of and ability to robustly apply the relevant legislation including the Human Rights Act, Equality Act, Mental Health Act, Mental Capacity Act and Care Act. With a grasp of local and national policy and procedures as well as an understanding of relevant case law and its application to complex social work interventions, you will have a strong ability to promote good practice in challenging contexts.

You will be adept at taking a person centred strengths-based approach and able to draw on a range of communication methods to support people to be involved in decision making. You will value working in partnership with important others and professionals. You will be skilled at using the perspectives of a range of disciplines to inform assessments and to assess and manage risks.

Being keen to think outside of the box to mitigate risks, you will bring a professional curiosity to practice, a focus on community resources, innovation and technology to meet the needs of those we support in the least restrictive way.

You will be supported by frequent high quality supervision and peer and leader support daily, alongside development opportunities.

As this role is public facing, applicants will need to demonstrate, at interview, their competency to converse and provide advice and guidance to members of the public, in spoken English to CEFR level C2: Can express him/herself spontaneously at length with a natural conversational flow, avoiding or backtracking around any difficulty so smoothly that the person with whom they are conversing is hardly aware of it.

At the moment due to the impact of the ongoing COVID-19 pandemic, some of our staff are working remotely from home but a return to a combination of working remotely and physically from the Council’s offices is anticipated in the near future.

As a new starter, your contractual location will be the designated council office for your team. Your working arrangements will vary according to your service and role, and the current Government guidance. For many roles, there will be more opportunities in the future for remote and hybrid working. However, this comes with the expectation that, as workplaces open up, there is likely to be an ongoing and occasional requirement to attend the workplace.
