Our Community Learning Disability Teams are on hand to find out how we can help. Whether offering advice and support or providing formal support, the goal is to make people’s lives easier and pleasant. To make sure we’re consistently lifting the burden and solving challenges, every service user is assessed and reviewed annually.

The Role:

Joining us in this new role, you’ll really enhance the specialist brokerage capacity in our Community Learning Disability Teams. It’s a varied job which spans everything from managing hospital discharges for Transforming Care patients, through to preventing hospital admission for those in the community who have high needs. Here, you can really get to know clients in person.

Fundamentally, your role is about obtaining value for money within the contractual arrangements behind nursing, residential and supported living care packages. You’ll provide the brokerage and placement function for complex and specialist care packages. And you’ll proactively support providers to improve the quality of their services and how they deliver.

Crucially, you’ll strengthen the market by reducing our reliance on specialist providers and reducing the costs of placements. The Learning Disability Service will value your support in ensuring residential and nursing placements are managed efficiently. That will include case-managing referral processes to ensure suitable placements are sourced and completed.

About You:

To join us, you’ll need a good understanding of adult social care including nursing, residential care and supported living. Experience of placements and care brokering is vital. Ideally, you will have experience relating to people with learning disabilities.

Naturally, you’ll need to be literate, numerate and confident negotiating costs. You’ll demonstrate the ability to work independently, researching suitable and effective solutions for clients. Good knowledge of Microsoft Excel and Mosaic Case Management System is essential. Equally important is your ability to build relationships of trust with service providers and families through empathetic, clear, honest and level-headed communication.
