Following an exciting and ambitious programme of change within Adult Social Care, we are currently looking for a Senior Social Worker to join our Hospital Discharge Team, which primarily requires Working to the Discharge to Assess model ( D2A).
As a Senior Social Worker you will have the opportunity to support and develop our incredible team of Social Workers, promoting positivity and encouraging wellbeing practices, as well as working closely with our passionate Team Manager, making this role an integral part of the overall team.
We are looking for someone who can work well within our friendly and supportive team and who is able to bring a flexible and pro-active approach to this role. This role will also involve working collaboratively with the wider Social Work Teams as well as Health colleagues and other key partners.
