About the role

Reporting to the Director of Integrated Adult Health and Social Care, this is your opportunity to provide the leadership that will be key in the implementation and delivery of our Alliance plan, whilst championing the transformational delivery of integrated health and social care services across the West of the county.

About the team

The West team has plenty of ambition and has been at the forefront of a number of exciting developments and projects over a number of years. We have established Integrated Teams between Adult Social Care and Community Health which are led by an Integrated Director post; we have a track record of delivering great practice pushing forward our Signs of Safety practice model and we work tirelessly to enable more and more people to be as independent as possible.

We are keen to do more. We are forever learning and ambitious to push further and further forward with kindness, compassion and determination. We want to use the opportunities that will flow from the newly formed Integrated Care System (ICS) to forge even further forward with integration that is in the best interest of the people we serve; we will always focus on quality and good customer outcomes, and we want to continue our work with our care providers to develop ever-better models of care, work which has seen new complex care models established and reablement focused care home approaches put in place.

About you

We are looking for an exceptionally strong and experienced operational leader with a clear track record of success in partnership working, with an emphasis on integration. You will be a professionally registered qualification in social care is required e.g. Occupational Therapy or Social Work. A strong social care operational background is also essential.

Able to demonstrate innovation to shape the future of adult health and social care services in the West of the county, ensuring that we make a real difference to the lives of residents.

Travelling requirements for your role

We positively encourage the use of technology to communicate, but in this role you will need to travel to locations away from your contractual base, so you must either hold a full and current driving licence and have access to personal transport or meet the mobility requirements of the role through other reasonable and suitable means.
