An exciting opportunity to be part of a brand new pilot project which aims to use the base principles of Reablement by enabling, maximising independence and a providing a preventative service for those with Mental Health difficulties.

In Kensington and Chelsea, we care about our diverse and vibrant community of residents, visitors, and visiting professionals. That’s why we’re pleased to have recently been granted funding from the Central and North West London NHS trust to create a brand new Mental Reablement Team. 

Reablement can be defined as a time limited, strengths-based model of care. It can be used as both a preventative service and a rehabilitative service. This team will have a specific focus on supporting people who were recently in hospital receiving treatment for mental health conditions, but who are now ready to return to the community. To support patients as an alternative to hospital admission with partnership with other services. 

Regardless of how complex or severe a patient’s individual mental health condition may be, adjusting to life outside of hospital may seem daunting and feel overwhelming. The Mental Reablement Team will provide a service that acts as a bridge between hospitals and the community, guiding and supporting people as they take the first few steps towards recovery and independence. 
 

The Role:

We’re looking for an Independent Living Assessor to help coordinate support and set goals for patients using the Mental Health Reablement service. You will work across Kensington, Chelsea, and Westminster – reviewing the progress each patient makes, and ensuring that they are able to live as independently and safely as possible while undertaking various activities in their day to day life.

Your work will have a strong focus on how to regain and maintain independence, which in turn reduces the need for long-term support. There will be externally provided support workers to help develop and deliver the new system alongside your team. You will be expected to work closely with both the Mental Health Reablement Service providers and external organisations supporting it throughout the length of your contract.

About You:

The Mental Health Reablement team will need to deliver an integrated, people-centred service that focuses on strength and rehabilitation.
Because of this, we’re looking for someone who has knowledge and understanding of the essential role of care coordination and Trusted Assessor within the setting of health or social care. The ideal candidate will have excellent communication skills, as well as the vision and drive to help establish this new service and make an impact. They will need to embody our values of respect, integrity, collaboration, and inclusiveness.

You must have a GCSE or equivalent in both Maths and English, as well as a Diploma or NVQ Level 3 in Health and social care or a related field.
