An exciting opportunity has arisen with a vacancy with Wakefield’s Adult Social Care’s Connecting Care West for a full time permanent Care Coordinator. Connecting Care West is a welcoming friendly team of Social Workers and Care Coordinators.
We work with people aged 18 and over with a social care need, or an appearance of need. We are looking for enthusiastic individual to join our team, ideally the successful candidates will have experience or knowledge in completing Care Act Assessments, Care Planning, reviewing and managing a busy case load.
You will be expected to support with our Urgent Response Team (URT), either supporting as a backup worker, or a full time URT worker dealing with urgent support as needed. The successful candidate will also need to have up to date knowledge of the Care Act 2014 and be open to improving their knowledge through training.
