This is an exciting opportunity for Social Workers to join the Community Mental Health Team in Bolton, that provide a comprehensive multi-disciplinary service. The Bolton CMHT is an Integrated Health and Social Care service that strives to deliver a recovery focused, trauma informed, needs based service for people with severe and enduring mental health conditions.

We are a friendly and supportive MDT team who value working with people from diverse backgrounds and cultures. The Functional CMHT’s are part of Bolton Division of Greater Manchester Mental Health NHS Foundation Trust and provide a comprehensive multi-disciplinary service to people aged 18 years and above, including an assertive outreach approach. We therefore welcome flexible and creative working to meet the needs of the people we support. We would welcome the opportunity to discuss this role with you.

About You

We are looking for individuals who are flexible, creative, committed and able to adapt positively to change. You should also be able to work effectively taking a holistic approach to assessment and treatment, working in partnership with service users and their carer’s promoting independence and recovery of people with serious and enduring mental health difficulties. You will need to be able to work effectively as a member of a multi-disciplinary team, respecting the input of all the professions and professionals within this.

About Us

As a Social Worker within in the Bolton CMHT you will assess service users using identified assessments, risk assessments and undertake the role of Care Coordinator in line with CPA process.

We are committed to promoting staff wellbeing and you will be offered supportive clinical supervision, monthly team training sessions for CPD ‘Lunch and Learn’, weekly staff mindfulness sessions via Microsoft Teams and the opportunity to develop skills in psychosocial interventions.

You will have a defined complex caseload which could consist of people who are on the Assertive Outreach pathway and will be expected to work proactively and flexibly within a multi-disciplinary team model. You will also be part of the extended hours service, which will incur weekend and bank holiday working a rota basis.
