This is an exciting opportunity to join the Older Adult Community Mental Health Team in Bolton.

The Older Adults CMHT function is part of Bolton’s Division of Greater Manchester Mental Health NHS Foundation Trust and provides a comprehensive multi-disciplinary service to the people of Bolton.

We are a friendly and supportive team who value working with people from diverse backgrounds and cultures. The ability to develop, maintain and improve relationships with service users is essential in this role as well as the attributes required to participate in inter-disciplinary team working. 

You maybe involve preparing for and participating in decision-making forums and multi-disciplinary meetings. These tasks should be completed in accordance with legislation, policies, procedures and best practice.

You will have a defined complex caseload and will be expected to work proactively and flexibly within a multi-disciplinary team model to achieve the best outcomes for our service users.

Experience of working within the Care Programme Approach framework and recovery model is desirable.

About You

We are seeking to recruit a specialist social worker who will provide advice and consultation, assessment and support to carers, staff, voluntary and independent sector organisations who provide care to older adults with organic and functional mental disorders in Bolton.

The post holder will be a member of a Multi Disciplinary Team providing expert advice to promote independence and ensure appropriate provision of treatment and care.

The post holder will work within the CPA process and Recovery model in addition to the Council’s personalisation agenda with a particular focus on supporting the timely assessment and discharge planning from the older adult’s mental health in-patient wards.

The service operates 365 days per year therefore the post holder will be required to work weekends and bank holidays on a rota basis.

AMHP qualification is desirable but not essential. The successful candidate must be willing to undertake AMHP training.
