An exciting opportunity has arisen for a Reablement Support Worker. You will be based within the community supporting residents in their home. This is a full-time post, 35 hours per week.

The Reablement Team is an integrated team of Health and Social care staff who work with Greenwich residents in their own homes to promote and maximise health and wellbeing to enable people to gain or regain their confidence, skills, and abilities to live as independently as possible.

As a Reablement Support Worker you will be required to flexibly support individuals within their own home and wider community. You will be expected to deliver direct care to residents of Greenwich and support with all aspects of daily living this includes supporting with personal care, prompting medication, meal preparation, supervise exercise plan devised by the therapy team.

You will be required maintain the safety and dignity of clients within their own homes and report any safeguarding concerns to your manager with an aim to protect clients from risk of abuse or neglect.

You will be required to provide comprehensive feedback and complete the client information documents at each visit.

You will be required to work and support residence of Greenwich with achieving their reablement goals.

Ideally, you will have experience providing care and support , have a positive attitude and passion for support people.

You will be well supported with regular supervision, and you will be given support to develop into the role by your manager and a commitment to working in a learning culture.
