This is an exciting time to join Sandwell Metropolitan Brough Council Adult Social Care.
We are looking to recruit Social Workers for a Borough wide Hospital & Intermediate Care Team. We operate a 7-day service and work Bank Holidays

You will work from various locations and will be responsible for supporting the timely discharge of people from hospital into intermediate care settings. You will be required to work in partnership with the NHS and the ICB and will have strong customer focus.
We use strengths and asset-based approach in our practice; how individuals can be supported by the strengths they have in themselves, their network of support from family, friends and their local communities. Where care and support is required we work to ensure that this reflects the individual’s assessed needs and strengths while promoting their independence, choice and control.

We are looking for committed and hardworking qualified Social Worker with Adult Social Work experience working with people with older people, people physical disabilities and sensory impairments, as well as mental health issues. You will need to be motivated and flexible with high professional standards and sound knowledge and experience of current legislation including the Care Act 2014, Safeguarding, Continuing Health Care, Mental Health Act, Mental Capacity Act 2005, and Community DOLS/LPS.
You will be expected to work with vulnerable adults and their carers with an asset-based and strength based approach to support them to achieve their outcomes within the wider community. You will need to be confident in undertaking safeguarding investigations, mental capacity assessments, best interest decisions, carrying out outcome focused assessments, with support planning, to meet their care and support needs.
You must be able to work with a diverse group of individuals and with our partner agencies to deliver the best outcomes for our customers and their carers.
You will be given an induction, appropriate training opportunities and regular supervision with an experienced manager. You will be working in a friendly environment, skilled, diverse and supportive team.
You must have a degree in Social Work, Dip SW, CQSW or equivalent, along with Social Work England registration.
The successful candidate will be required to undertake an Enhanced DBS check.
